Google Forms has been updated since the creation of this video, however, you can still use this method to create level 1 evaluations. With this, you can capture how your eLearning users feel about the training they’ve just completed. This video was recorded with Adobe Captivate 8, however much of the procedures remains the same with newer versions of Captivate.

Summative assessment is often difficult. This is a great idea.
