Software Simulation Assessment is only tracking one click box when taking the assessment.

I am experiencing a problem with Captivate 12.4. I have created an assessment for a software simulation and it’s only tracking 1 of the click boxes even though I have enabled reporting (include in quiz, add to total, and report to LMS are all checked for every check box in the slides). There are 14 click boxes, however, the assessment seems to only be tracking/adding 1 of the clicks (I cannot figure out which one). It does see that there are 14 questions (clicks). Any suggestions on how to resolve this issue would be appreciated?
NOTE: I just did some testing and the only point it is counting is a slide where text must be typed in. It is not counting slides where just a click is made into the final score, but is including them in the total points for the assessment.
